HOW TO GET YOUR MEDICAL CARD

After you receive doctor's paper work, get picture of 3 pages, picture of you and picture of your driver's license, then go to https://tap.dor.mt.gov/_/#1 and fill in all needed information, add needed pictures and pay $20 fee to state as last step.
Here is the instruction how to fill information for medical card.
You will get your temporary card by e-mail immediately. Its good for 60 days. You can print it and make purchase the very same day. During next 10 days you will get physical card in your mail box which is good for 1 year.

Beginning 2 June 2020 any registered card holder is allowed to purchase marijuana products from any registered medical marijuana provider.
For getting your first medical marijuana card or if you need to renew old one, please download PHYSICIAN STATEMENT for a DEBILITATING MEDICAL CONDITION here. It is state paper work your primary care physician should do.
If you have any of these condition you can apply to get you Medical Marijuana Card:
☐ Cancer, glaucoma or positive status for human immunodeficiency virus, or acquired immune deficiency syndrome when the condition or disease results in symptoms that seriously and adversely affect the patient’s health status
☐ Cachexia or wasting syndrome
☐ Severe chronic pain that is persistent pain of severe intensity that significantly interferes with daily activities as documented by the patient’s treating physician
☐ Intractable nausea or vomiting
☐ Epilepsy or an intractable seizure disorder
☐ Multiple sclerosis
☐ Crohn’s disease
☐ Painful peripheral neuropathy
☐ A central nervous system disorder resulting in chronic, painful spasticity or muscle spasms
☐ Admittance into hospice care
☐ Post-traumatic stress disorder (PTSD)
